Fertility Tracking & Planning
Brief Consultation Video Chat - No Fee
Preconception Visit, $250
For those choosing this path but who are not yet pregnant and have additional questions or desire to meet in-person for support, planning and discussion: Meet with Shelly Varelli LM, CPM and/or Erin CNM, APRN for 30-60 minutes, includes with 3 months of follow-up care (does not include midwifery care for pregnancy),
Initial visit: Review and discussion of your medical history. Review ways to boost your optimal health to achieve pregnancy, education and support on cycle tracking. Discussion and donor options, resources, and ways to boost fertility through diet and lifestyle.
Evaluation of fertility charting and timing, $150
For those needing extra support and education to understand fertility, and how to track cycles.
Phone and email availability during business hours for discussion. Clients can expect a response to contact within 48-72 hours.
● In-person or video call: Review and discuss cycle tracking, review recent health, review and discuss diet, stress, and lifestyle, and provide support for fertility-promoting changes, ordering and evaluating lab work, and assist in creating a new plan to support fertility and achieve pregnancy
● One additional 15-minute follow-up: Evaluation of charting timing, labs, etc, phone and email availability during business hours for discussion. Clients can expect a response to contact within 48-72 hours.
Intrauterine Insemination
Service is provided in our West Toledo based office, with a scheduled appointment. Be sure to fill out the inquiry form above.
IUI, first in cycle: $350 for washed sperm, add $50 for unwashed sperm requiring preparation
IUI, second in same cycle: $150 washed sperm, add $50 for unwashed sperm requiring prep
IUI Hours of Service: 8 am-6 pm
Unless requested by the midwife, IUI services outside regular hours incur an additional fee of $25
Service Area: in-home service within 30 minutes of Toledo, Ohio. Clients outside of the service area may receive services in our office or have the option of paying a travel fee for home care of $50
